August national G+ pattern
In the August National Survey Preparedness Brief, F0689 appeared at G+ for 498 active facilities across 500 Standard-flagged citation rows in the 15-month window.
Active facilities with an F0689 citation at G+.
Standard-flagged F0689 citation rows at G+.
Of those 500 rows, CMS recorded 81 as past noncompliance already corrected before the survey. The figures describe published citation records, not 500 unresolved current conditions.
The review should start where the record has to connect incident follow-up, supervision, interventions, wounds, and cross-disciplinary review.
Run the review like a trace, not a meeting
Incident to intervention
Take one recent fall, elopement risk, or accident-related event and trace investigation, root cause, care plan update, communication, and monitoring.
Intervention to practice
Confirm the intervention is visible across shifts and resident-specific enough to defend.
Practice back to record
Make sure the record supports what staff say is happening now, not only what was planned.
