What We Collect

We collect account information such as email address, authentication identifiers, selected home facility, subscription status, regional access requests, Scout Report submissions, moderation status, and basic audit records needed to operate the service.

Public Source Data

Facility and survey activity in Survey Alert comes from public CMS and Medicare.gov Care Compare inspection information. We use this data to show Care Compare observed activity, CMS confirmation pending status, CMS confirmed records, 90-day Complaint activity, and 15-month Standard survey history.

Scout Reports And PHI

Do not submit protected health information, resident names, medical record details, room numbers, staff names, contact information, or other private personal information in Scout Reports. Scout Reports are intended for tactical survey-preparation context, not patient details or personnel allegations.

We review Scout Reports before broad publication and may reject, edit, remove, or retain reports for moderation, abuse prevention, audit, and safety reasons.

Published Scout Reports are anonymous to other operators. SignalCoreIQ keeps internal account, facility, moderation, and audit records so we can review submissions, prevent abuse, resolve support issues, and maintain contribution history and support moderation.

Operator Intel includes anonymous nearby Scout Reports and Operator Questions. Both use the same 25-mile home-facility access boundary. Questions and answers must remain anonymous to other operators while retaining internal moderation and abuse-control records.

How We Use Information

We use information to authenticate users, connect accounts to facilities, enforce access rules, show survey activity, operate Operator Intel moderation, manage subscriptions, troubleshoot service issues, prevent abuse, and improve product reliability.

Optional Analytics And Attribution

Survey Alert asks before optional Google Analytics can load in this browser. You can allow or decline analytics and change that choice using Privacy choices on our public pages. We remember your choice for up to 180 days when browser storage is available. Essential account, payment, and report access features work with either choice. Advertising storage and personalization remain disabled.

Campaign attribution is not currently retained, and report, account, facility, and purchase events are not sent to marketing analytics. We do not send names, email addresses, facility identifiers, Scout Report or Operator Question content, author identity, protected health information, or other free text to marketing analytics.

Declining analytics prevents the Google tag from loading. Changing from Allow to Decline stops analytics, clears accessible Google Analytics cookies and may reload the page. Your account and purchase records are separate from this browser choice.

Sharing

We do not sell personal information. We may share information with service providers that help operate Survey Alert, when required by law, or when needed to protect the service, users, or the public.

Account Deletion

Signed-in users can initiate account deletion from the account deletion page. Some records may be retained when needed for billing, security, moderation, legal compliance, or abuse prevention.

Security

We use access controls, server-side authorization checks, moderation workflows, and audit records to reduce privacy and abuse risk. No online service can guarantee perfect security, so please report suspected issues promptly.

Report Purchases And Delivery

For report purchases, we process the checkout or account email, report selections, payment-provider references, and delivery and access records to fulfill purchases, provide account access and support, and prevent duplicate fulfillment or abuse. Guest purchases use the checkout email to establish access. Do not share temporary access links.

Stripe processes payments. Hosting, storage, authentication, and email providers support report delivery and access. Public CMS facility records are separate from buyer account information. Operational purchase records do not change your optional analytics and attribution choices.

Purchased report access lasts 12 months. After access expires, files remain private while a separate deletion schedule is reviewed; we do not promise automatic deletion at that point or automatic restoration of access. Purchase, billing, security, and support records may be retained for those purposes or applicable legal obligations. Contact support about account deletion and related purchase records.
