What We Collect

We collect account information such as email address, authentication identifiers, selected home facility, subscription status, regional access requests, Scout Report submissions, moderation status, and basic audit records needed to operate the service.

Public Source Data

Facility and survey activity in Survey Alert comes from public CMS and Medicare.gov Care Compare inspection information. We use this data to show Care Compare observed activity, CMS confirmation pending status, CMS confirmed records, 90-day Complaint activity, and 15-month Standard survey history.

Scout Reports And PHI

Do not submit protected health information, resident names, medical record details, room numbers, staff names, contact information, or other private personal information in Scout Reports. Scout Reports are intended for tactical survey-preparation context, not patient details or personnel allegations.

We review Scout Reports before broad publication and may reject, edit, remove, or retain reports for moderation, abuse prevention, audit, and safety reasons.

How We Use Information

We use information to authenticate users, connect accounts to facilities, enforce access rules, show survey activity, operate Scout Report moderation, manage subscriptions, troubleshoot service issues, prevent abuse, and improve product reliability.

Sharing

We do not sell personal information. We may share information with service providers that help operate Survey Alert, when required by law, or when needed to protect the service, users, or the public.

Account Deletion

Signed-in users can initiate account deletion from the account deletion page. Some records may be retained when needed for billing, security, moderation, legal compliance, or abuse prevention.

We use access controls, server-side authorization checks, moderation workflows, and audit records to reduce privacy and abuse risk. No online service can guarantee perfect security, so please report suspected issues promptly.
