Professional Use
Survey Alert is built for skilled nursing facility administrators, directors of nursing, operators, and related compliance teams. Use the service for operational awareness and preparation, not as a substitute for legal advice, clinical judgment, CMS guidance, or official survey agency communications.
Data And Status Labels
Survey Alert aggregates public CMS and Medicare.gov Care Compare inspection information. Complaint fast alerts reflect recent Care Compare observations in a 90-day window. Standard survey history is reconciled against CMS Provider Data Catalog sources on a monthly cadence and shown in a 15-month window.
Labels such as Care Compare observed, CMS confirmation pending, and CMS confirmed describe the source status available to Survey Alert. They do not represent a government endorsement, final legal determination, or complete survey record.
Scout Reports Acceptable Use
Scout Reports are for practical, survey-related context that could help another care team prepare. Do not submit PHI, resident names, staff names, contact information, room numbers, medical-record details, accusations, profanity, threats, discriminatory or derogatory language, spam, or promotional content.
SignalCoreIQ may review, edit, reject, remove, or limit Scout Reports that do not meet these rules or that create safety, privacy, quality, or abuse concerns. Approved Scout Reports may help build contribution history. Scout Reports are free within 25 miles of your home community and do not change billing or access.
Published Scout Reports are anonymous to other operators and are shown as nearby context rather than exact public posting locations. SignalCoreIQ still keeps internal account, facility, moderation, and audit records for review, abuse prevention, support, and contribution history.
Operator Intel, including anonymous nearby Scout Reports and Operator Questions, follows the same 25-mile access boundary measured from your home facility. Operator Questions and answers must use the same moderation, anonymity, and location-protection rules when available.
Accounts And Access
You are responsible for keeping your account credentials secure and for using access only within 25 miles of your home facility. Do not attempt to bypass geographic entitlements, rate limits, moderation, or administrative controls.
Billing, Cancellation, And Refunds
Survey Alert web subscriptions are managed through Stripe. You can review plan status, start checkout, or open Stripe billing management from the Billing page. Cancellation and refund details are available in the linked policies.
No Official Partnership
Survey Alert is independently operated by SignalCoreIQ. It is not operated by, endorsed by, or formally partnered with CMS, Medicare.gov, Care Compare, or any state survey agency.
